I wanted to post an update.  Ko isn't doing well.. We had the vet out again yesterday, and his best guess is something in his system is shutting down. All of the blood work came back like a healthy young horse, so it's just so confusing. He's still alert, showing no signs of pain and still flirts with the girls when they walk by.
